Diabetic ulcers are complex wounds that require specialized care. Proper wound care is crucial to prevent amputation, and one effective treatment option is negative pressure wound therapy. However, the cost of negative pressure wound therapy can often be a barrier, making it difficult for caregivers and families to access.

In recent years, the number of patients with ischemic skin ulcers due to diabetes mellitus and arteriosclerosis obliterans are increasing. Accordingly, endovascular therapy, drugs, and various wound dressings have been developed and applied to diabetic foot ulcers, and negative-pressure wound therapy, which often requires expensive and burdensome procedures for medical personnel, has also become popular.

Serratia marcescens causing a soft tissue infection is very rare. But, among the immunocompromised patients it is becoming an important pathogen causing infection. Here we report a case of an elderly male with type 2 diabetes mellitus presenting with severe soft tissue infection due to S. marcescens. Infection complicates the pathological picture of diabetic foot and plays a main role in development of moist gangrene. A total sample size of 100 patients with diabetic foot ulcer was included in this cross sectional study. A detailed history was obtained and assessment of clinical symptoms and signs were made. Discharge from the lesions, pus aspirates from the abscesses and debrided necrotic materials were collected. Aerobic culture anerobic culture and antibiotic susceptibility testing was done. Males were the predominant group in this study 66% as compared to females 34%.

Diabetic foot ulcer is a major complication of diabetes mellitus, and probably the major component of the diabetic foot. Wound healing is an innate mechanism of action that works reliably most of the time. The present study was carried out in Department of Microbiology, in a tertiary care hospital, over a period of one year from June 2016 – May 2017. 50 diabetic patients with foot ulcer attending the Surgery Out Patient Department were included in the study. These patients were diabetic or detected to be diabetic by blood glucose estimation at the time of their visit to the hospital.

With the fact that melanoma can mimic a wide variety of neoplasms, one also needs to be careful while dealing with the aspirates from multiple sites; particularly, in the absence of a reliable history, as it is not infrequent to find a coexisting second lesion with melanoma. In this context, we share the experience of one of our recent cases with a previous history of melanoma of foot. This patient presented with inguinal lymphadenopathy, as well as, an intra-abdominal mass.

lization patients transition to a removable cast walker, followed by permanent prescription footwear or bracing (135, 543). Mean time from surgery to therapeutic shoes has been reported to be about 27 weeks (7 months) (135, 140, 530). Careful patient selection and management is the rule with these complex diabetic cases, since amputation can be a complication of failed surgical procedures (138, 474, 511, 527, 528, 533).

Microbiology S. aureus is a pathogen in more than half of cases of contiguous-focus osteomyelitis. However, in contrast to hematogenous osteomyelitis, these infections are often polymicrobial and are more likely to involve gram-negative and anaerobic bacteria. Hence a mixture of staphylococci, streptococci, enteric organisms, and anaerobic bacteria may be isolated from a diabetic foot infection or pelvic osteomyelitis underlying a decubitus ulcer.
