Caregivers and patients with such chronic illnesses experience economic, physical, social and psychological distresses which may lead to chronic sorrow. Chronic sorrow is viewed as a normal reaction to loss, however it can progress to a pathological state such as depression if the coping styles are ineffective.

Accelerated oxidative damage is one of the hallmarks in both sickle cell disease (SCD) and thalassemia major (TM). A decreased antioxidant level is found in both diseases. Our study was carried out to evaluate the variation in serum levels of Selenium and Vitamin E among a group of transfusion dependant Egyptian SCD and TM patients, further more to correlate these levels with iron overload status or transfusion requirements.

Active renewal policies that required families to submit documentation to verify their continued eligibility were associated with substantial disenrollment. Requiring active eligibility redetermination every 6 months rather than every 12 months resulted in even higher levels of disenrollment over time. Up to one- quarter of children who disenrolled at renewal returned to SCHIP within 3 months (18 percent to 27 percent).

Most SCHIP enrollees lived in families with a working adult (80 percent to 87 percent of enrollees, depending on the study State) who worked full time (63 percent to 74 percent). Although the study States permitted coverage for families with incomes up to as much as 250 percent of the FPL, nearly two-thirds to three-quarters of new enrollees lived in families with incomes equal to or below 150 percent of the FPL. Most new SCHIP enrollees were white (55 percent to 78 percent), but a significant proportion of SCHIP enrollees were black or Hispanic.

The early identification and treatment of toxic stress, including child maltreatment, can lessen the associated long-term negative health and behavioral outcomes. Daycare providers, teachers, and other adults who interact frequently with children should have sufficient knowledge and skills to identify and care for children who have been exposed to traumatic childhood experiences. They should be familiar with support services to meet the needs of children whose problems cannot be adequately addressed by front-line staff.
