The rs2681492, a single nucleotide polymorphism in the ATP2B1 gene, has been reported to be associated with cardiovascular diseases and traits including blood pressure, chronic heart failure, and hypertension. The study aimed to develop a PCR-RFLP genotyping method for rs2681492 among the Vietnamese population.

Cardiac hypertrophy (CH), leading to cardiac failure is due to chronic metabolic alterations occurring during cellular stress. Besides the already known relationship between oxidative stress and CH, there are implications of reductive stress leading to CH. This study attempted to develop reductive stress-based CH rat model using n-acetyl-L-cysteine (NAC), a glutathione agonist that was compared with typical isoproterenol (ISO) induced CH model.

Atrial natriuretic peptide (ANP), via its guanylyl cyclase A (GC-A) recep-tor and intracellular guanosine 3¢,5¢-cyclic monophosphate production, is critically involved in the regulation of blood pressure. In patients with chronic heart failure, the plasma levels of ANP are increased, but the car-diovascular actions are severely blunted, indicating a receptor or postrecep-tor defect.

Nonimmunologic Mechanisms Nonimmunologic mechanisms that protect against pneumonia include filtration of air as it passes through the nasopharynx, the glottal reflex, laryngeal closure, the cough reflex, clearance of organisms from the lower airways by ciliated cells, and ingestion by pulmonary macrophages and PMNs of small bacterial inocula that manage to reach alveolar spaces. Respiratory virus infection, chronic pulmonary disease, or heart failure compromises these mechanisms, predisposing to the development of pneumococcal pneumonia.

Primary (Spontaneous) Bacterial Peritonitis Peritonitis is either primary (without an apparent source of contamination) or secondary. The types of organisms found and the clinical presentations of these two processes are different. In adults, primary bacterial peritonitis (PBP) occurs most commonly in conjunction with cirrhosis of the liver (frequently the result of alcoholism).

Approach to the Patient: Dyspnea (Fig. 33-3) In obtaining a history, the patient should be asked to describe in his/her own words what the discomfort feels like, as well as the effect of position, infections, and environmental stimuli on the dyspnea. Orthopnea is a common indicator of congestive heart failure, mechanical impairment of the diaphragm associated with obesity, or asthma triggered by esophageal reflux. Nocturnal dyspnea suggests congestive heart failure or asthma.
