The world's disease profile is changing, and chronic diseases now account for the majority of global morbidity and mortality. The causes of chronic kidney diseases (CKD) reflect this change. The increase in prevalence of CKD is partly due to a real increase in its frequency (due to the increase in the average age of people surviving), better detection of CKD, but also to the increasing incidence of diabetes and hypertension among people today, not only within the developed world, but also increasingly within the emerging world(1) .

Acute kidney injury causes great morbidity and mortality in both the community and hospital settings. Understanding the etiological factors and the pathophysiological principles resulting in acute kidney injury is essential in prompting appropriate therapies. Recently hyperuricemia has been recognized as a potentially modifiable risk factor for acute kidney injury, including that associated with cardiovascular surgery, radiocontrast administration, rhabdomyolysis, and associated with heat stress.

A growing body of literature argues that the availability and mix of physician specialties in areas is important for health outcomes. Areas with fewer specialists but higher generalists per capita were found to have better health outcomes or quality of care [21,22]. Goodman [23] found that greater physician supply is associated with both higher area income and lower mortality rates, and argued that regional variations in health outcomes and physician supply will exist as long as there are differences across communities in economic status.

Clusters of cancer, birth defects, and other chronic illnesses have sometimes been linked to chemicals or other toxic pollutants in local communities, although these links can be controversial. There is a need for better documentation and investigation of disease clusters to identify and address possible causes.
