We present a case of primary biliary cholangitis diagnosed during pregnancy. Diagnosis of this entity in pregnancy is infrequent, and when everything seemed to point to a simple obstetric cholestasis, close attention to the details of the clinical history was required to raise suspicion of the true diagnosis.

Sigmoid volvulus in pregnancy is a rare cause of intestinal obstruction with high maternal and fetal morbidity and mortality if not diagnosed and managed early. Early diagnosis and management can prevent adverse effects such as bowel ischemia and preterm labor. Because classic clinical and radiological features may not be evident, high degree of suspicion is warranted.

Diagnosing self-limited conduction abnormality of Lyme carditis in absence of pathognomonic skin rash or history of tick bite is challenging but necessary to avoid placement of pacemaker particularly in young patients. High degree of clinical suspicion, rapidly progressing conduction block and prompt response to antibiotics may help in diagnosis.

Clinically hypomyopathic dermatomyositis is a rare disease that is important to recognize, investigate and treat early as it is associated with poor prognosis. In a proportion of patients, myositis specific antibodies could be negative, but with high clinical suspicion, myositis associated antibodies should be ordered.

MRI is considered to be the diagnostic tool of choice in diagnosing nerve root compromise among patients presenting with clinical suspicion of lumbo-sacral radiculopathy. There exists controversy among researchers and clinicians regarding the diagnostic utility and accuracy of MRI in detecting nerve root compromise and radiculopathy.

Neonatal sepsis refers to systemic and generalized infection of the newborn which is documented by a positive blood culture in the first four weeks of life. It is one of the four leading causes of neonatal mortality in India, to isolate bacterial pathogens in neonates with clinical suspicion of sepsis from blood samples and to study their antimicrobial susceptibility pattern. Blood samples collected from 1072 neonates with suspected sepsis were processed. Out of 1072 neonates 231(21.54%) were culture positive.

This study was carried out to identify and phenotypically characterise SSIs due to these organisms at a tertiary care centre. Antibiotic susceptibility was ascertained using microbroth dilution. Strict Infection control measures were then put into place to prevent these infections. Infections due to these organisms require prolonged treatment and occasionally even surgery. It is important to have a high index of suspicion to be able to recognise these infections and to identify them in a clinical microbiology laboratory.

This study was undertaken to determine the prevalence of fungal agents involved in chronic rhinosinusitis (CRS) and to analyse its clinico-mycological profile. Fifty patients with clinical suspicion of CRS attending a tertiary care hospital during a 6 months period were included in this analysis.

The objective of our study was to know the efficacy of the tests like Leptocheck WB IgM(ICT), PanBio IgM ELISA(ELISA), with MAT. Hundred and eleven serum samples from 54 patients with clinical suspicion of leptospirosis and 50 serum from asymptomatic subjects was tested for IgM antibodies by the kits. Out of 54 serum collected, 42.59% were positive by ICT, 48.14% by ELISA, and 53.7% by MAT.

Myiasis, the tissue invasion of living vertebrate animals by the fly larvae is commonly observed throughout tropical regions of the world. The phenomenon is also witnessed among humans where unhygienic conditions are prevalent. We reported one case of 29 years old female associated with hard palate perforation who had complaints of whitish worm like structure creeping out of her nose and mouth. On examination, Lacrimal fistula noted in lacrimal sac area of right eye in which maggots were found. The larva isolated was that of third instar larvae Chrysomya bezziana (Old World screwworm).

The study revealed, benign theileriosis, which is caused by Theileria orientalis conformed by using the PCR/ Blood smear/ Lymph node aspiration smear examination. The incidence of theileriosis was 24.21% to the total cases recorded in the veterinary hospital. Based on the clinical suspicion the incidence rate is 90.91%. The incidence rates of theileriosis in female animals (85%) were higher than male (15%) animals. Age 3-4 years (34%) and 5-6 years (20%) higher the Theileria incidence rate than the younger animals (9%).

A 2-year prospective study was carried out on 110 children with clinical suspicion of meningitis where C-reactive protein (CRP) determination and routine cytochemical and microbiological analysis of Cerebrospinal fluid (CSF) were done for all patients. The patients were divided into four groups: Pyogenic meningitis (PM), viral meningitis (VM), tubercular meningitis (TBM) and control groups.

Diagnosis and management of patients with tuberculous pleuritis needs a rapid and sensitive diagnostic tool. Our aim was to diagnose cases of tuberculous pleuritis from patients employing ZN smear, culture (LJ & MGIT) and to compare with PCR. Pleural fluid collected from 36 patients with clinical and/or radiological suspicion of TB pleuritis was included in this study. Pleural fluid collected from nine non-tuberculous pleuritis cases was included as negative controls.

Bacteriological confirmation plays a key role in the diagnosis of tuberculosis. Many systems have been developed in the recent years apart from traditional microscopy and culture on Lowenstein Jensen (LJ) medium. The objective of this study is to compare the rate of detection of Mycobacteria by acid fast staining, LJ medium and Mycobacteria Growth Indicator Tube (MGIT). Hundred patients with clinical suspicion of pulmonary tuberculosis attending a tertiary care hospital over a period of one and a half years were enrolled in the study.

It has been observed that the efficacy of dipeptidyl peptidase-4 (DPP-4) inhibitors as compared to the placebo groups in some clinical trials conducted in China is weaker than that in trials conducted outside China, leading to the suspicion that this may be caused by differential Glycosylated Hemoglobin (HbA1c) response in the placebo arm of DPP-4 inhibitor clinical trials conducted in China compared to other countries.
