Common bile duct stone

Residual lithiasis is the presence of stones in the common bile duct, ignored after one or more biliary interventions. We report an atypical case of chronic symptomatic lithiasis of the lower bile duct occurring 41 years after biliary surgery, managed successfully by ideal choledochotomy.

During the three years (2015-2017), 68 patients having CBD stones concomitant gallstones suitable criteria for inclusion at Hue Central Hospital. Retrospective clinical description study. Results: the average age is 52.2±12.5 years (24-90) and the male/ female ratio is 0.7/1 (27/41). Abdominal pain was the most common symptom 91.2%, which was followed by jaundice 51.5%; direct bilirubin increased by 27.3±15.6 μmol/l (2.2-165). The size of CBD stones is 12.4±3.2 mm (6-20), the size of gallstones is 11.3±6.2 mm (5-36). The first time CBD stones 95.6%, recurrent CBD stones 4.4%.

Laparoscopic repair used to be contraindicated to the patients who had a history of abdominal operation in general and bile duct surgery in particular. Additionally, the development of techniques in medicine together with surgeon’s advanced experience has allowed an expansion of laparoscopic repair in the treatment of cholelithiasis, especially for the cases of reoperation.

To evaluate the role of laparoscopic surgery in the treatment of intrahepatic biliary stones, an area that is not well-studied. Subjects and methods: A descriptive crosssectional, retrospective study was carried out on a total of 56 patients. Results: 56 patients with intrahepatic biliary stones underwent laparoscopic surgery. The average age was 54.1 ± 12.17. The number of patients with biliary stones in over 2 segments of liver was 62.5%. The average time of operation was 145 ± 20.37 minutes. The average time of exploration of bile duct was 61.52 ± 33 minutes.

Cholangitis refers to an acute infection of the biliary tree, and has the potential to cause significant morbidity and mortality. It occurs secondary to stasis or obstruction of bile compounded by the presence of bacteria. Choledocholithiasis (stones in the common bile duct) has long been the most common cause of obstruction, however, strictures, tumors, or manipulation of the common bile duct may cause bile stasis, leading to the predisposition to bacterial infection of the biliary tree.
