The paper analyzes changing demographics and changes for old age support in Vietnam, and suggests some solutions for the issues, notably: a more coherent approach to old-age support, deeper consolidation of delivery systems in the interests of efficiency, transparency and client friendliness, more humane and fiscally sustainable aged and long-term care systems built around a system of home- and community-based care. The role of government will remain paramount both in short and long terms, but non-state providers would gradually be expected to player more of such a role.

Although at that point, the challenges of a potential Pandemic Influenza were not being considered, this risk is now also being addressed by the training. Avian Influenza has continued its march across Europe and remains a serious threat (Revill 2007; Shaikh 2007). In this survey, 91% of senior living (SL) and long term care (LTC) administrators felt ill-prepared to deal with public health emergencies and BT threats. Eighty percent of the respondents reported that their LTC communities did not have any training (either educational or exercise based) for their workforce in this area.

Several community health centers operate a PACE program, a home and community based service that allows severely disabled elders who are eligible for nursing home placement to remain in the community. PACE is usually based in adult day health centers and operates as a small Medicare Advantage capitated managed care plan at risk for providing all Medicare and Medicaid covered services including long term care and acute hospital care. Primary care services are also provided by the PACE program in a clinic setting utilizing employed or contracted medical providers.

Health centers play varied roles in relation to nursing homes, from following existing patients, to having their physician serve as medical director of nursing homes, to owning and/or operating nursing homes. Community health centers that partner with skilled nursing facilities (SNFs) can enhance the well being of their patients, the community and their organizations. The health center may provide a range of services to help a patient remain in the community, but at times some patients will enter a SNF.

ADHC is a community-based health and long term care service aimed at elders or adults who are disabled enough to be in a nursing home or at risk of nursing home placement. When coordinated with other health center services, particularly primary care clinic services, ADHC can be critical in allowing elders to avoid nursing home placement and helping informal caregivers to continue providing care over an extended period. Participants live at home and are brought into the center from 3 to 5 days a week.

Non-state providers including commercial firms, not-for-profit organisations and faith-based organisations often provide services when governments are unable to meet people’s SRH needs. Social franchising, or networks of private providers who offer a standard set of services and share training, referral systems, quality standards and brands is one such example. The high volume of patients that these networks can provide for enables them to reduce costs of treatment for poor people.

In this paper, we consider the extent to which these quality of life issues are supported by governmental action in Canada and four comparison nations. The information relevant to these issues comes primarily from two types of data sources: indicator analyses from international reports and intensive and detailed policy analyses of two policy issues of particular importance to women: childcare provision and gov- ernmental support for community-based long-term care. Canadian data are contrasted with those from Den- mark, Sweden, the UK, and the US.

Long-term care providers are supported either locally, by the federal states, or by non-profit or private organisations. A supply contract (Versorgungsvertrag) is concluded between these institutions and the insurance funds. This contract is essential for ongoing home care or nursing home care in that it qualifies this form of support for the recognised LTC market. The supply contract regulates the type, contents, and extent of the general nursing benefits which a care institution must provide. It also defines the so-called care package (Leistungskomplexe).
