Decongestion is an important primary goal in acute heart failure treatment. Loop diuretics remain a fundamental role in congestion management and symptom improvement in patients with acute heart failure. Diuretic combination contributes to achieving better diuretic efficacy. However, the optimal timing for diuretic combination in clinical practice has not been distinctly recommended in current guidelines.

Nonimmunologic Reactions Fluid Overload Blood components are excellent volume expanders, and transfusion may quickly lead to volume overload. Monitoring the rate and volume of the transfusion and using a diuretic can minimize this problem. Hypothermia Refrigerated (4°C) or frozen (–18°C or below) blood components can result in hypothermia when rapidly infused. Cardiac dysrhythmias can result from exposing the sinoatrial node to cold fluid. Use of an in-line warmer will prevent this complication.

Hyponatremia in the setting of ECF volume expansion is usually associated with edematous states, such as congestive heart failure, hepatic cirrhosis, and the nephrotic syndrome. These disorders all have in common a decreased effective circulating arterial volume, leading to increased thirst and increased AVP levels. Additional factors impairing the excretion of solute-free water include a reduced GFR, decreased delivery of ultrafiltrate to the diluting site (due to increased proximal fractional reabsorption of Na+ and water), and diuretic therapy.

Twenty years ago in the twenty-first edition of the Principles and Practice of Medicine, the authors described what was then the practice for the pharmacologic therapy of patients with heart failure, which included digoxin and a diuretic [1]. In addition, the authors noted that recent studies had supported the potential use of vasodilators in the treatment of this population of patients.
