Endobronchial tuberculosis

Current data for the utility of radial endobronchial ultrasound (EBUS) in investigating peripheral lung lesions (PLLs) has been restricted to populations with low pulmonary tuberculosis (TB) incidence. The aim of this study was to assess the diagnostic utility of radial EBUS with guide sheath in the diagnosis of peripheral lung lesions in Singapore, a high TB incidence setting.

Tracheobronchial stenosis is a known complication of endobronchial tuberculosis. Despite antituberculous and steroid therapy, the development of bronchial stenosis is usually irreversible and requires airway patency to be restored by either bronchoscopic or surgical interventions.

Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is widely used to perform mediastinal lymph node sampling. However, little information is available on polymerase chain reaction for Mycobacterium tuberculosis (TB-PCR) using EBUS-TBNA samples in patients with intrathoracic granulomatous lymphadenopathy (IGL).

Tuberculosis as a primary cause of respiratory failure is an uncommon occurrence1 with an incidence of 1.5% in patients hospitalized with pulmonary TB2. Patients with miliary or disseminated disease are especially prone to develop respiratory failure. Tuberculous Pneumonia has rarely been identified as a cause of ARF3-4. Acute tuberculous pneumonia presents as parenchymal consolidation with or without endobronchial spread mimicking bacterial pneumonia.
