Lung cancer diagnosis is usually achieved through a set of bronchoscopic techniques or computed tomography guided-transthoracic needle aspiration (CT-TTNA). However these procedures have a variable diagnostic yield and some patients remain without a definite diagnosis despite being submitted to an extensive workup.

Small lung nodule is a common problem in pulmonary practice. The definition of a classical solitary pulmonary nodule is a single, spherical, well-circumscribed, radiographic opacity less than or equal to 30 mm in diameter that is completely surrounded by aerated lung and is not associated with atelectasis, hilar enlargement, or pleural effusion. Ideally, the goal of diagnosis and management is to promptly bring to surgery all patients with operable malignant nodules while avoiding unnecessary thoracotomy in patients with benign disease.

Fine-Needle Aspiration Cytology/ Biopsy (FNAC/FNAB) is now a widely accepted diagnostic procedure, which has largely replaced open biopsy. This method is applicable to lesions that are easily palpable, for example swellings in Thyroid, Breast, superficial Lymph node etc. Imaging techniques, mainly ultra-sonography and computed tomography, offer an opportunity for guided FNAC of deeper structures. The practice of diagnostic cytology needs proper training of the laboratory personnel including cytopathologist, cytotechnologist and cytotechnician.

Various needles were used to collect tissue for cytological or histological diagnosis. Franzen et al. developed a fine needle and guide for prostatic aspiration by the transrectal route (figure 8). The Franzen needle and guide were designed to allow accurate needle placement into the abnormal area palpated by the fingertip. It was secured by the metal ring fixed to the fingertip and a plate in the palm of the hand. A rubber fingerstall was pulled over it and a 23- or 25-gauge needle was used. Up to six passes could be made in one session (Berner and Orell, 2010).
