Gynecological hysterectomy

At the time of benign gynecological surgery, a prophylactic salpingo-oophorectomy or salpingectomy is increasingly being performed concurrently to reduce the risk of future ovarian and fallopian tube cancer. We herein describe a case of hereditary breast and ovarian cancer syndrome in which a hysterectomy and bilateral adnexectomy were performed with a preoperative diagnosis of benign tumor.

Uterine leiomyoma is a benign gynecological tumor of the uterine myometrium layer in the female reproductive system, with clinical incidence in approximately 25% of women. In rare instances, it is possible for it to grow into a giant uterine leiomyoma (weighing 11.4 kg or more). Surgery on a giant mass is challenging because the enlargement mass affects the normal anatomical position of the internal genital organs.

Uterine myomas are the most common benign tumors in women, affecting 20%–50% of reproductive age population. Myomas cause significant morbidity and are the single most common indication for hysterectomy in the United States, representing a major personal and public health concern worldwide. Recent research on the cellular and molecular biology of myomas has enabled us to understand better the pathogenesis and pathophysiology of this tumor, but more remains to be done.

If a patient has previously undergone a hysterectomy, the cervix is usually no longer present and only a vaginal cuff remains. If the pathology was benign, then the patient no longer requires Pap smears (11). However, if the hysterectomy was performed for cervical cancer or dysplasia, cervical cancer screening on the vaginal cuff should continue since remnants of cervical tissue may be present. These women are also at higher risk for vaginal in- traepithelial neoplasia (VAIN) and vaginal cancer.

About one-third of American women will eventually have a hysterectomy, the second most common surgery after cesarean section. Why so common? Basically, hysterectomy can cure a variety of uterine problems: ✿ Fibroids. Hysterectomy is most often recommended for serious cases of fibroids (benign tumors that grow in the uterus wall). Fibroids can grow large and may cause great pain and heavy bleeding. Depending on the severity, physicians and patients may decide to try treatments other than hysterectomy. In a surgery called myomectomy, the fibroids are removed but the uterus stays in place.
