Transportation project expenditures create jobs and business activity directly. An economic analysis tool called Input-Output Tables (http://en.wikipedia.org/wiki/Input- output_model) is used to quantify the direct and indirect jobs and business activity created by specific expenditures by tracking how dollars flow from one industry to another within a particular jurisdiction, such as a region or country. Care is needed when interpreting this information since the data are aggregated and do not necessarily reflect the specific program or project being considered.

In addition to this paradigm shift from batch and queue to single-piece flow, Lean Manufacturing requires a systematic elimination of all possible forms of non-value-added costs (e.g., waste). In essence, pollution is a manifestation of economic waste and is a sign of production inefficiency, revealing flaws in product design or production processes. It is the unnecessary, inefficient, or incomplete utilization of a resource, or represents a resource not being used to its highest value.

This book is about change in the management of public services – how much of it and what consequences. For over two decades the goal of restructuring welfare provision has been at the heart of UK government policy. Under the Conservatives the focus was on controlling expenditure and re-organising services to make professionals more accountable for resource decisions. In health, education and social care, the objective was to install a system of managed provision heavily influenced by the practices of private firms.

The tuberculosis (TB) epidemic is historically anchored in South Africa’s Western Cape province, and long precedes the HIV epidemic. This is despite the generally better socio-economic status of the population and greater overall healthcare expenditure in this region than in other provinces. Contributing factors include overcrowding, alcohol abuse, poverty, and administrative neglect of the National TB Programme (NTP).1 This traditional ‘Cape’ epidemic is now being significantly augmented by an HIVassociated TB epidemic.

Overweight and obesity in the United States among adults and children has increased significantly over the last two decades. Those following typical American eating and activity patterns are likely to be consuming diets in excess of their energy requirements. However, caloric intake is only one side of the energy balance equation. Caloric expenditure needs to be in balance with caloric intake to maintain body weight and must exceed caloric intake to achieve weight loss (see tables 3 and 4).
