Clear cell sarcoma of the kidney is an uncommon pediatric renal malignant neoplasm that is typically characterized in 2–3-year-olds by aggressive behavior and late relapses. Until recently, surgical resection was considered the only option for treatment of kidney cancer, especially renal cell carcinoma. The disease is relatively resistant to both radiotherapy and chemotherapy, and although alternative systemic therapies such as interleukin- 2 immunotherapy and interferon have shown promise, objective response rates are still quite low. Renal Cell Carcinoma Renal cell carcinomas account for 90–95% of malignant neoplasms arising from the kidney. Notable features include resistance to cytotoxic agents, infrequent responses to biologic response modifiers such as interleukin (IL) 2, and a variable clinical course for patients with metastatic disease, including anecdotal reports of spontaneous regression. Epidemiology The incidence of renal cell carcinoma continues to rise and is now nearly 51,000 cases annually in the United States, resulting in 13,000 deaths. The male to female ratio is 2:1.

Neoplasms (See also Chap. 374) Back pain is the most common neurologic symptom in patients with systemic cancer and may be the presenting symptom. The cause is usually vertebral metastases. Metastatic carcinoma (breast, lung, prostate, thyroid, kidney, gastrointestinal tract), multiple myeloma, and non-Hodgkin's and Hodgkin's lymphomas frequently involve the spine. Cancer-related back pain tends to be constant, dull, unrelieved by rest, and worse at night. By contrast, mechanical low back pain usually improves with rest.
