To determine the inter-and intraobserver reliability of MRI for diagnosis of lumbar far lateral disc herniation. Method: 27 patients in whom lumbar far lateral disc herniation was definitely diagnosed by operative findings at our department between May 2018 and December 2022 were included.

Discal cyst is a rare disease, the pathogenesis is not yet clear and its symptoms are very similar to lumbar disc herniation. Although some cases may regress spontaneously, most cases of lumbar discal cysts are treated surgically.

A nerve root anomaly, typified by a conjoined nerve root, is a rare finding. Conjoined nerve root anomalies are easily missed even in preoperative advanced imaging modalities, which can be potentially troublesome during and after surgery. In this report, we present a case of conjoined right L5–S1 nerve root in a patient with lumbar disc herniation, accompanied by spina bifda occulta, which was undiagnosed on preoperative imaging studies.

Sciatica pain is a typical symptom of lumbar disc herniation (LDH), but some neurogenic and malignant tumours surrounding the sciatic nerve can also cause similar symptoms. These tumours are often misdiagnosed or even mistreated as LDH in clinical practice. In our clinical practice, we found two patients with malignant tumours who were misdiagnosed with LDH. One patient complained of pain and numbness in the right lower limb.
