Lack of access to risk-appropriate maternity services, particularly for rural residents, is thought to be a leading contributor to disparities in maternal morbidity and mortality. There are several existing measures of access to obstetric care in the literature and popular media. In this study, we explored how current measures of obstetric access inform the number and location of additional obstetric care facilities required to improve access.

Sigmoid volvulus in pregnancy is a rare cause of intestinal obstruction with high maternal and fetal morbidity and mortality if not diagnosed and managed early. Early diagnosis and management can prevent adverse effects such as bowel ischemia and preterm labor. Because classic clinical and radiological features may not be evident, high degree of suspicion is warranted.

Uterine rupture is a rare complication that can occur in the first trimester of pregnancy. It can lead to serious maternal morbidity or mortality, which is mostly due to catastrophic bleeding. First-trimester uterine rupture is rare; hence, diagnosis can be challenging as it may be confused with other causes of early pregnancy bleeding such as an ectopic pregnancy.

Current research is focused on clinical, institutional/policy level deficiencies with little community involvement and not much is said about culture, beliefs and practices that may negatively impact on maternal health. Under the current Primary Health Care model, community involvement is represented by a Community Health Committee (CHC) made of a chief, police officer, health professional and a school principal.

Malnutrition and poor health status especially in rural areas is a common problem. It retards children growth, increases the risk and duration of illness, reduces work output, and slows social and mental development. Malnutrition among women of reproductive age increases the risk of mortality during labor and delivery and puts their newly born children at risk of long-term deficiencies. Improving nutritional status, including micronutrient status, can lead to increased productivity, increased child survival and growth, and reduced maternal morbidity and mortality.

Access to skilled birth attendance is one measure of inequity in maternal health. Differences in socioeconomic aspects account for about two-thirds (61%) of inequalities in skilled birth attendance, of which household wealth accounts for 24%, mother’s education 16% and living in rural areas 12%. 12 When examining the relationship between utilization patterns and offer of skilled care during delivery, women who did not receive valid antenatal care (at least four visits to a health professional during pregnancy) were more likely to deliver without professional care.
