A case report is presented, in which a tooth with a chronic apical abscess that has caused a maxillary sinusitis was treated with an endodontic microsurgery. A 65-year-old female patient reported discomfort in a maxillary tooth gingiva. Clinically, a dental metal-ceramic bridge with adequate marginal adaptation, involving teeth from maxillary right canine to maxillary right second molar, was observed.

Nasotracheal intubation is a very useful technique for orofacial or dental surgery. However, the technique itself can be more traumatic than that of orotracheal intubation. Dentists and oral surgeons perform tooth extractions. If the tooth is impacted, they cut away gum and bone tissue which cover the tooth and then, using forceps, grasp the tooth and gently rock it back and forth to loosen it from the jaw bone and ligaments that hold it in place. Once the tooth has been extracted, a blood clot usually forms in the socket. Sometimes, the blood clot in the socket breaks loose, exposing the socket leading to dry socket. But when this wok of tooth extraction is performed by unauthorized and unskilled persons there is breach in protocols.

The aim of this study is to analyze the maxillary sinus dimensions both linearly and volumetrically using cone beam computed tomography (CBCT) to assess the maxillary sinus pneumatization. Retrospective analysis of 30 maxillary sinuses belonging to 15 patients’ CBCT scans was performed. Linear and volumetric measurements were conducted and statistically analyzed.

The maxillary sinus continues to grow until eruption of the permanent teeth (4). The adult sinus is variable in its extension, in one-half of cases extending into the alveolar process forming an alveolar recess. In these instances the sinus comes in close proximity to the roots of the maxillary posterior teeth. With the loss of posterior teeth, the sinus can extend further into the alveolar bone, even reaching the alveolar ridge occasionally (4).

The maxillary sinuses are one of the four paired sets of the paranasal sinuses and the first to develop. In the second month of fetal life, an invagination forms in the lateral wall of the nasal fossa in the middle meatus (1). This study characterized maxillary sinus mucositis (SIMS) adjacent to teeth with apical periodontitis (AP), and assessed its resolution three months after endodontic treatment. 29 subjects who maxillary posterior teeth with AP were imaged with cone-beam computed tomography (CBCT). Resolution of SIMS was assessed with CBCT three months after treatment, and periapical healing was assessed using the PAI after six months. Four non-compliant subjects were discontinued and SIMS was identified in 14/25 subjects (56%).
