Cardiac glycosides are natural compounds used for the treatment of congestive heart failure and cardiac arrhythmias. Recently, they have been reported to exhibit anticancer activity. Proscillaridin A (PSN-A), a cardiac glycoside constituent of Urginea maritima has been shown to exhibit anticancer activity. However, the cellular targets and anticancer mechanism of PSN-A in various cancers including prostate cancer remain largely unexplored.

The past thirty years have witnessed the birth, growth, and evolution of clinical electrophysiology from a field whose initial goals were the understanding of arrhythmia mechanisms to one of significant therapeutic impact. The development and refinement of implantable devices and catheter ablation have made non-pharmacologic therapy a treatment of choice for most arrhythmias encountered in clinical practice.

Glossopharyngeal Neuralgia Syncope due to glossopharyngeal neuralgia (Chap. 371) is preceded by pain in the oropharynx, tonsillar fossa, or tongue. Loss of consciousness is usually associated with asystole rather than vasodilatation. The mechanism is thought to involve activation of afferent impulses in the glossopharyngeal nerve that terminate in the nucleus solitarius of the medulla and, via collaterals, activate the dorsal motor nucleus of the vagus nerve.

The pathophysiology of cardiac arrhythmias is complex and the actions of drugs that are useful in stopping or controlling them may seem equally so. Nevertheless many patients with arrhythmias respond well to therapy with drugs and a working knowledge of their effects and indications pays dividends, for irregularity of the heart-beat is at least inconvenient and at worst fatal. The mechanisms by which the failing heart may be sustained are now better understood; carefully selected and monitored drugs can have a major impact on morbidity and mortality in this condition.
