The evidence for epigenome-wide associations between smoking and DNA methylation continues to grow through cross-sectional studies. However, few large-scale investigations have explored the associations using observations for individuals at multiple time-points. Here, through the use of the Illumina 450K BeadChip and data collected at two time-points separated by approximately 7 years, we investigate changes in methylation over time associated with quitting smoking or remaining a former smoker, and those associated with continued smoking.

A novel fnding related to bulla and bleb formation in patients with primary spontaneous pneumothorax
Spontaneous pneumothorax is a common problem globally. Bullas and blebs have been implicated in this problem, but the etiology of their formation is unknown. We aim to show the relation between a novel clinical finding, the pulmonary delayed inflation (PDI) sign, and the etiology of bulla and bleb formation in young patients.

We previously showed that human papillomavirus (HPV) serostatus was not an independent risk factor for esophageal squamous cell carcinoma(ESCC) in nonsmokers and nondrinkers; however, HPV increased the risk in smokers.

The associations between microRNAs and lung cancer have received increasing attention. This study assess the association between polymorphisms in miR-135a-2, miR-219-2 and miR-211 genes and the risk of lung cancer, as well as the gene–environment interaction between these polymorphisms and cooking oil fume exposure.

Denmark. Boffi et al. (2006) measured OTS respirable particle pollution in a car park (open space), outdoors in front of a conference center with smokers under a roof (18 smokers during a measurement time of 35 min), indoors in the nonsmoking conference center, along the motorway to Copenhagen city centre, and inside a Copenhagen restaurant where smoking was allowed. Although passive exposure to tobacco smoke has been linked to a number of health problems, it is unresolved whether it alters breast cancer risk. Most, but not all, studies that compared women who were passively exposed to tobacco smoke to women with no exposure to tobacco smoke reported an association of passive smoking with an increased risk of breast cancer. Only two of these studies showed a “dose- relationship”, where an increase in breast cancer risk was related to more tobacco smoke exposure.

A comprehensive search of the scientific literature for this report is complete through September 1991. In addition, pertinent studies published through July 1992 have been included in the analysis in response to recommendations made by reviewers. Due to both resource and time constraints, the scope of this report has been limited to an analysis of respiratory effects, primarily lung cancer in nonsmoking adults and noncancer respiratory illnesses in children, with emphasis on the epidemiologic data.

Cancers of the oropharynx often metastasize to upper and middle jugular chain lymph nodes (Levels II and III), but can also spread to retropharyngeal lymph nodes, which distinguishes them from oral cavity tumors and must be considered when treating oropharyngeal cancers. Tumors in this site are generally treated with radiotherapy, as a single modality for T 1/2 or N 0/1 stages. Increas- ingly, some of these cancers are associated with human papilloma virus 16 infection, especially in nonsmokers.

A healthy 45-year-old man is found on routine screening to have hypertriglyceridemia. He is a nonsmoker, has a reasonable diet, consumes one alcoholic drink per week, and exercises regularly. He takes no medications. His father died at the age of 55 years in an automobile accident; his mother is healthy at 67 years of age, and he has two healthy older brothers. His blood pressure is normal, his body-mass index (the weight in kilograms divided by the square of the height in meters) is 28, and his waist circumference is 96 cm.

Roxanne Nelson May 28, 2008 — Periodontal disease is associated with a small but significant increase in cancer risk, researchers report. The association between periodontal disease and overall cancer risk was found in both smokers and nonsmokers, according to a study that appears in June issue of Lancet Oncology. The authors note that periodontal disease might be a marker of a susceptible immune system or might directly affect cancer risk.

The following three examples introduce the subject of clinical reasoning: A 46-year-old man presents to his internist with a chief complaint of hemoptysis. The physician knows that the differential diagnosis of hemoptysis includes over 100 different conditions, including cancer and tuberculosis. The examination begins with some general background questions, and the patient is asked to describe his symptoms and their chronology.
