Hemostasis is the dynamic equilibrium between coagulation and fibrinolysis. During pregnancy, the balance shifts toward a hypercoagulative state; however placental abruption and abnormal placentations may lead to rapidly evolving coagulopathy characterized by the increased activation of procoagulant pathways.

Every two years, the WHO convenes an Expert Committee on Selection and Use of Essential Medicines to review state- of-the-art evidence on the efficacy, safety, and cost effectiveness of medicines in order to update the WHO Model Lists of Essential Medicines (WHO EML) for adults and children. Countries are encouraged to use this model list to inform the development of their own national EMLs. In the 17th edition of the WHO EML, oxytocin, misoprostol, and magnesium sulfate are included. 16 Oxytocin is included for prevention and treatment of postpartum hemorrhage.

Previous chapters emphasized physiologic concepts concerning cardiac and vascular function at the cellular and organ level. In addition, they examined mechanisms, such as baroreceptors and circulating hormones, that regulate overall cardiovascular function. This chapter integrates all the components of the cardiovascular system and shows how they work together to maintain normal perfusion of organs under conditions of increased organ demand for blood flow (e.g., during exercise and pregnancy) or during abnormal stressful conditions such as hemorrhage....

Leptospirosis is an emerging infectious disease of global importance, as illustrated by recent large outbreaks in Asia, Central and South America, and the United States. The disease is caused by pathogenic leptospires and is characterized by a broad spectrum of clinical manifestations, varying from inapparent infection to fulminant, fatal disease. In its mild form, leptospirosis may present as an influenza-like illness with headache and myalgias. Severe leptospirosis, characterized by jaundice, renal dysfunction, and hemorrhagic diathesis, is referred to as Weil's syndrome.

The publication of this textbook marks a great milestone in our joint efforts at achieving the Millennium Development Goal 5, which aims to reduce maternal mortality by three-quarters, a daunting task indeed for most developing countries. The African region, especially Eastern and Western Africa, has the highest ratio of women dying as a result of pregnancy or childbirth in the world, estimated at an average of 1000 per 100 000 live births.

Failure of the uterus to contract and retract following childbirth has for centuries been recognized as the most striking cause of postpartum hemorrhage. Uterine atony is a condition which, in spite of the presence of effective medical interventions, still claims thousands of maternal lives. In the developing world, lack of access to uterotonic therapies that have been available for almost a century represents one of the most glaring disparities in obstetric care today.

The key feature of all resuscitation efforts is the early recognition and management of hypovolemic shock with an overall objective of restoring the circulating blood volume to maintain normal tissue perfusion and oxygenation. The reason for the high mortality associated with obstetric hemorrhage is simple, i.e. the delayed recognition of hypovolemia and failure to provide adequate volume resuscitation. Common problems include the failure to recognize risk factors, frequent under-estimation of the degree of blood loss, and failure to involve key personnel early enough.
