Orthostatic hypotension

Intact orthostatic blood pressure regulation is essential for early mobilization after surgery. However, postoperative orthostatic hypotension and intolerance (OI) may delay early ambulation. The mechanisms of postoperative OI include impaired vasopressor responses relating to postoperative autonomic dysfunction.

Patients admitted to intensive care units (ICU) are often treated with intravenous (IV) vasopressors. Persistent hypotension and dependence on IV vasopressors in otherwise resuscitated patients lead to delay in discharge from ICU. Midodrine is an oral alpha-1 adrenergic agonist approved for treatment of symptomatic orthostatic hypotension.

This is the first case report description, to our knowledge, of a cranial mediastinal mass (suspected thymoma) causing orthostatic hypotension in a dog. A Labrador Retriever presented for urethral stent placement during cystoscopy secondary to transitional cell carcinoma diagnosis. Introduction: Phenothiazine drugs, including chlorpromazine and levomepromazine, have been being widely used as neuroleptics (major tranquilizers), antiparkinsonian drugs and antihistaminics for a long time [1]. Table 1.1 shows chemical structures of representative phenothiazines. These drugs show blocking action on D2 receptors of dopaminergic neurons; there is close relationship between the receptor blocking and tranquilizing actions. The dopamine D2 receptor-blocking actions provoke extrapyramidal symptoms, such as muscular stiffness, tremor and ptyalism.
