Demographic and epidemiological transitions are changing the disease burden from infectious to noncommunicable diseases (NCDs) in low- and middle-income countries, including Bangladesh. Given the rising NCD-related health burdens and growing share of household out-of-pocket (OOP) spending in total health expenditure in Bangladesh, we compared the country’s trends and socioeconomic disparities in financial risk protection (FRP) among households with and without NCDs.

The United States has seen major advances in medical care over the past decades, but access to care at an affordable cost is not universal. Many Americans lack health care insurance of any kind, and many others with insurance are nonetheless exposed to financial risk because of high premiums, deductibles, co-pays, limits on insurance payments, and uncovered services. One might expect that the U.S. poverty measure would capture these financial effects and trends in them over time.

High medicines prices, low affordability and poor availability are key impediments to access to treatment in many low- and middle-income countries (1–9). Certainly, in those countries where the majority of the population still buys its medicines through out-of-pocket payments, the high cost of medicines (relative to the household budget) means that an illness in the family exposes that family to the risk of catastrophic expenditure. Too often the choice is made to go without.

Data gleaned from the various NHA surveys were used to estimate household expenditures. A household survey was not conducted for this NHA exercise and, despite several requests to contacts at the Living Conditions Monitoring Survey (LCMS), that survey was not available to the NHA team. 1 Table 2 shows the distribution of household out-of-pocket payment directly made to public and private providers. Households made 31.9 percent of health expenditures at public providers, 65.8 percent at private providers, and 2.4 percent at pharmacies.
