Epigenetic modulation plays a vital role in regulating the transcriptional machinery of human genome in response to infection. Differential Methylation pattern of the genome especially the retro transposon repetitive elements (RE) which constitutes more than 45% of the genome had been studied to trace the impact of RE methylation in the disease severity.

Extra-pulmonary tuberculosis (EPTB) is a milder form of disease in terms of infectivity as compared to pulmonary TB (PTB). Whereas sputum can be easily obtained for the detection of disease in lungs, diagnosis of EPTB is often difficult requiring invasive and expensive serological/radiological investigations. A category-wise drug treatment is similar for the two forms of disease1. However, an assessment of end point of cure is a problem with EPTB.

Current tuberculosis (TB) reporting protocols are insufficient to achieve the goals established by the Stop TB partnership. Some countries have recommended implementation of active case finding program. We assessed the effect of Cough Officer Screening (an active screening system) on the rate of TB detection and health care system delays over the course of four years. Methods: Patients who were hospitalized at the Changhua Christian Hospital (Changhua, Taiwan) were enrolled from September 2004 to July 2006 (Stage I) and August 2006 to August 2008 (Stage II).

Small samples and study area covering only 2 DOTS centers are some of the study limitations. Other limitations include exclusion of testing for latent TB infection (LTBI). This was as a result of shortcomings associated with use of the only available diagnostic tool- Tuberculin (Mantoux) skin test (TST). These include invasive nature of the test, the need for the patient to come back for follow up TST reading, cross-reactivity with environmental mycobacteria and previous BCG vaccination.
