Second-hand smoke is a severe health hazard for children. Clinical guidelines suggest that nurses advise smoking parents to quit when they accompany their sick children to paediatric settings, but the guidelines did not mention what nurses can do if the parents are not with the children.

Secondhand smoke (SHS) exposure of children due to parental tobacco use is a particularly prevalent health issue and is associated with adverse health outcomes. Following the US Clinical Practice guidelines, pediatricians in the United States deliver 5A’s (ask, advise, assess, assist, and arrange) counseling to smoking parents which has proven to be effective.

The recruitment process for an intervention program addressing SHS exposure in children was considerably more challenging and time consuming than anticipated. Barriers at both a parents level and a doctor’s level can be discriminated.

Even though some children will remain among the dwindling ranks of the uninsured, the new health law brings the number of uninsured children in this country to an unprecedented low. It accomplishes this feat in several ways. First and foremost, it includes a requirement that all children, as well as all adults, have health insurance. This new mandate begins in 2014, and shortly thereafter, phases in penalties over a few years to ensure that parents are signing their children up for coverage and signing up for coverage themselves.

Oral health professionals play a significant role in counseling patients concerning the harmful effects of tobacco, alcohol and recreational drugs. During the pregnancy, the consequences of these behaviors are profound. Multiple studies have demonstrated a clear association between maternal smoking and perinatal morbidity and mortality (6-10). Women who smoke are at increased risk for low birth weight babies, bleeding during pregnancy, premature labor and preterm rupture of membranes.
