Few reports to date have evaluated the efects of diferent pedicle screw insertion depths on sagittal balance and prognosis after posterior lumbar interbody and fusion (PLIF) in patients with lumbar degenerative spon‑ dylolisthesis (LDS). A total of 88 patients with single-level PLIF for LDS from January 2018 to December 2019 were enrolled. Long screw group (Group L): 52 patients underwent long pedicle screw fxation (the leading edge of the screw exceeded 80% of the anteroposterior diameter of vertebral body).

We compared the perioperative results and complications associated with PLIF and TLIF, and collected evidence for choosing the better fusion method. The evidence indicated that TLIF could reduce the complication rate and durotomy. Neither TLIP nor PLIF was found superior in terms of clinical satisfaction or radiographic fusion rate. PLIF might result in longer time in surgery.

The risk of posterior cage migration (PCM) exists when a fusion cage is used for transforaminal lumbar interbody fusion (TLIF). This complication is influenced by contact pressure between the endplate and the cage. Previous reports demonstrated that anteriorly located cages bore more load and had greater strain than posteriorly located cages.

: Literature indicates that adjacent-segment diseases after posterior lumbar interbody fusion with pedicle screw fixation accelerate degenerative changes at unfused adjacent segments due to the increased motion and intervertebral stress. Sagittal alignment of the spine is an important consideration as achieving proper lordosis could improve the outcome of spinal fusion and avoid the risk of adjacent segment diseases.
