Research suggests that in some patients with low back pain, lumbar belts (LB) may derive secondary prophylactic benefits. It remains to be determined, however, which patients are most likely to benefit from prophylactic LB use, and which LB design is optimal for this purpose.

Chondroitin Sulphate (CS), a natural glycosaminoglycan of the extracellular matrix, has clinical benefit in symptomatic osteoarthritis but has never been tested in gout. In vitro, CS has anti-inflammatory and positive effects on osteoarthritic chondrocytes, synoviocytes and subchondral bone osteoblasts, but its effect on macrophages is unknown.

In medicine, decisions are made on a daily basis about the provision, withholding or withdrawing of treatments, many of which could prolong life. Treatments which could provide a therapeutic benefit are not inevitably given but are weighed according to a number of factors, such as the patient’s wishes, the treatment’s invasiveness, side effects, limits of efficacy and the resources available. Prevention studies: For women who have a family history of ovarian cancer, the risk of developing the disease may be reduced by removing the ovaries before cancer is detected. This surgery is called prophylactic oophorectomy. Women who are at high risk of ovarian cancer are taking part in trials to study the benefits and harms of this surgery. Other doctors are studying whether certain drugs can help prevent ovarian cancer in women at high risk.

Weighing the potential benefits, potential adverse events, and costs associated with antibiotic prophylaxis, the expert committee of the American Heart Association has dramatically restricted the recommendations for antibiotic prophylaxis. Prophylactic antibiotics (Table 118-7) are advised only for those patients at highest risk for severe morbidity or death from endocarditis (Table 1188).
