Propofol and etomidate

Over the last few decades intensive studies have been carried out on the molecular targets mediating general anesthesia as well as the effects of general anesthetics. The γ-aminobutyric acid type A receptor (GABAAR) has been indicated as the primary target of general anaesthetics such as propofol, etomidate and isoflurane, and sedating drugs including benzodiazepines and barbiturates.

The effects of the intravenous anesthetics propofol and etomidate on circulation are significantly different; however, their differing effects on miRNA expression in the cardiovascular system are not clearly understood. The purpose of this study is to investigate the effects of these two anesthetics on miRNA expression profiles in the heart and blood vessels.

Twelve dogs of either sex age group of 7-14 years, presented to the college clinic with surgical problems were utilized to study the effect of etomidate and propofol after premedication with atropine sulphate, diazepam and fentanyl. The animals were divided into two groups of six animals each. Etomidate was given intravenously in group I dogs @ 0.3mg/kg b.wt. Propofol @ 6 mg/kg b.wt. intravenously was given in group II dogs. Induction quality was excellent, smooth and attained sternal recumbency rapidly without struggling in all animals in both groups.

Propofol may result in hypotension and respiratory depression, while etomidate is considered to be a safe induction agent for haemodynamically unstable patients because of its low risk of hypotension. We hypothesized that etomidate anesthesia during ERCP caused more stable haemodynamic responses compared with propofol.
