Sarcoidosis is a multisystemic granulomatous disease of unknown origin. It is characterized by abnormal activation of lymphocytes and macrophages with the formation of granulomas. Most cases have asymptomatic pulmonary involvement. In case of symptoms, they have an excellent response to glucocorticoid therapy.

Sarcoidosis is a disease of unknown etiology that is characterized by non-caseating granulomas in multiple organs. It can involve the lung in various forms. Nodular involvement is a rare form of pulmonary sarcoidosis which can be mistaken for malignancy. Diagnosis requires compatible clinical picture along with imaging findings suggestive of the disease and noncaseating granulomas on histopathology.

Thoracic computed tomography scan showed pulmonary nodules and mediastinal and hilar lymphadenopathy. Bronchoalveolar lavage revealed a CD4/CD8 ratio of 8, and lymph node biopsy confirmed noncaseating granulomas. The diagnosis of sarcoidosis with ocular, pulmonary, and neurologic involvement was made.

Cutaneous involvement in aspergillosis is an uncommon entity. It usually occurs secondary to hematogenous spread from pulmonary infections in immunocompromised patients. Primary infection occurs at site of skin injury following trauma, burn, surgery or intravenous cannulation. As per literature, cutaneous aspergillosis is rarely seen in immunocompetent patients. Common causes reported for cutaneous aspergillosis are A. fumigatus and A. flavus. Here we report an unusual case of 25 years old immunocompetent male with primary cutaneous aspergillosis.
