CCD is an autosomal dominant disorder caused by haploinsufficiency of Runt related transcription factor (RUNX2) essential for osteoblast cell commitment and chondrocyte maturation. A mutation of the RUNX2 (Core Binding Factor-α1 gene located at chromosome 6p21) is usually caused CCD. In general, the estimated prevalence of CCD is under diagnosed because of the relative lack of medical complications in comparison to other skeletal dysplasias. No available data on the prevalence of CCD or its characterization in Saudi families.

Reality shock negatively impacts on initial workplace transition, productivity, and ultimately, employee retention, hence it is important to ascertain its causative factors so that measures can be taken to mitigate its effects. The rationale for selecting three cardinal symptoms is based on their high sensitivity and their relatively high specificity for ABRS, especially when considering the time interval of persistence for 10 days or longer. 36-38 Purulent nasal drainage predicts presence of bacteria on antral aspi- ration when reported as purulent rhinorrhea by the patient, when manifest as postnasal drip or purulent discharge in the posterior pharynx, or when observed in the nasal cavity or near the sinus ostium. 39,40 Purulent rhinorrhea also predicts radiographic evidence of ABRS.

There are two situations in which the TST is not required. Applicants providing written documentation (with a health-care provider’s signature) of a TST reaction of 5 mm or greater of induration or applicants with a history of a severe reaction with blistering to a prior TST may be excluded from this requirement. Applicants in these two groups must undergo a chest radiograph. A verbal history of a positive TST reaction from the applicant is not acceptable. If the TST is not administered for one of these two reasons, the civil surgeon should not check the box next to “Tuberculin Skin Test”.

The physical examination in cardiogenic pulmonary edema is notable for evidence of increased intracardiac pressures (S3 gallop, elevated jugular venous pulse, peripheral edema), and rales and/or wheezes on auscultation of the chest.
