The second group was formed of men with a suspicion of prostate cancer based on an elevated serum PSA and/or abnormal digital rectal examination, and the blood sample taken immediately before the prostate biopsy. All men older than 40 years and attended at HOSCAR or DIPRECA, without a previous history of prostate cancer and fulfilled the criteria’s for prostate cancer screening or a prostate biopsy were invited to participate. With a gloved index finger inserted into the rectum with a Silverman biopsy needle applied close to the finger with the tip of the needle in line with the tip of the finger and the bevel edge facing away from the finger. The needle is then rotated half a turn so that the beveled edge is against the finger. This avoided an inadvertent needlestick injury. The abnormal area was palpated and the needle directed through the rectal mucosa and towards the area, but not into it.

Physical Findings Patients with early disease may not have any significant abnormalities detectable on physical examination. Jaundice may be a presenting feature in some; in these patients a palpable, nontender gallbladder (Courvoisier's sign) may be palpated under the right costal margin. Patients with more advanced disease may have an abdominal mass, hepatomegaly, splenomegaly, or ascites. The left supraclavicular lymph node (Virchow's node) may be involved with tumor, or widespread peritoneal disease may be palpable on rectal examination in the pouch of Douglas.
