Pyruvate kinase deficiency is an exceptionally rare autosomal recessive Mendelian disorder caused by bi-allelic pathogenic variants in the PKLR gene. It is mainly characterized by chronic nonspherocytic hemolytic anemia though other symptoms such as splenomegaly, hepatomegaly, pallor, fatigue, iron overload, shortness of breath, hyperbilirubinemia, and gallstones might also prevail.

Subcapsular hepatic hematoma is a rare and life-threatening complication of pregnancy. It is most commonly associated with severe preeclampsia and hemolytic anemia, elevated liver enzymes, and low platelets syndrome. Patients with subcapsular hepatic hematoma typically present with epigastric, right upper quadrant or shoulder pain, nausea and vomiting, and/or shortness of breath.

Historically, it has been found that malignancy is associated with superior vena cava (SVC) syndrome. The past decade has seen more cases of thrombogenic and stenotic SVC syndrome due to increased use of pacemakers and indwelling central lines. As compared to the slowly progressing obstruction in malignancy, rapid thrombogenesis rate and a lack of venous collateral sequelae lead to more acute sequelae in these patients.

Cardiac lymphoma is a rare neoplasm involving heart, pericardium or both, usually seen in immunocompromised patients. We report a 61-year-old male presenting with worsening shortness of breath and 20-pound weight loss. Investigations showed right-sided tumor with interatrial septal wall perforation and left atrial expansion.

Lymphangiomas are rare benign malformations of the lymphatics that occur due to blockage of the lymphatic system during fetal development. They commonly occur in the neck and axilla, while involvement of the pericardium is rare. We report herein the case of a 16-month-old Sri Lankan child with a large pericardial cystic lymphangioma presenting with sudden-onset shortness of breath.

Exogenous lipoid pneumonia is a rare disease caused by aspiration or inhalation of oily substances. Case presentation: A 66-year-old male with dry cough (Case 1) and a 38-year-old female with shortness of breath (Case 2) demonstrated ground-glass opacities on chest computed tomography and were diagnosed with lipoid pneumonia based on the confirmation of lipid-laden alveolar macrophages.

The present report a rare case of Salmonella paraytphi A causing pleural effusion in a 22 yr female patient who presented with shortness of breath, fever, and loose stool. It has been clinically diagnosed as Pleural effusion and pleural fluid was sent for culture and sensitivity. Using standard bacteriological tests, Vitek2 compact system and serologically, the isolate was confirmed to be Salmonella paratyphi A. Salmonella paratyphi A is a rare pathogen causing pleural effusion. Hence it needs to be considered under differential diagnosis when treating the case of pleural effusion.

Hemophagocytic lymphohistiocytosis (HLH) is an uncommon and life-threatening disorder that may rarely complicate the clinical course of Orientia tsutsugamushi disease (scrub typhus). Here, we describe the clinical features, laboratory parameters, management, and outcome of 16 children with scrub typhus-associated HLH. All patients satisfied the HLH-2004 diagnostic criteria. All patients had fever of unknown origin and multisystem damage. Raised hepatic transaminases and abnormalities in routine blood test were observed in all children. Imaging tests showed abnormalities in 10 cases.
