Splenic metastasis

Solitary metastasis of a malignancy to the spleen is rare, particularly for gastric cancer. Only a few case reports have documented isolated splenic metastasis from early gastric cancer. We describe a case of splenic metastasis from early gastric cancer.

Thymomas are typically slow-growing tumors and AB type thymomas are considered no/low risk tumors with a better prognosis. Extra-thoracic metastases are extremely rare. To the best of our knowledge, no patient with an isolated splenic metastasis from a thymoma has been reported. We report a patient who underwent laparoscopic splenectomy for a slow-growing, isolated splenic metastasis, eight years after thymectomy.

As splenectomy and spleen-preserving lymphadenectomy are performed only in some proximal gastric cancer patients, it is difficult to identify patients who have undergone radical gastrectomy with or without splenic hilar (No.10) or splenic artery (No.11) lymph node metastases.

The spleen is the intra-abdominal organ most often injured as a result of blunt trauma. The spleen is the most vascular organ of the body and, for this reason, splenic injury is potentially life threatening. The most common mechanism for such injury is motor vehicle collision. Left lower rib fractures are suggestive of spleen injury, although an intact rib cage does not exclude spleen trauma.
