Many studies show that hyperglycemia can lead to injury to the intestinal mucosal barrier, increasing the likelihood of infection, leading to intestinal inflammation in people with type 2 diabetes. Recently, investigations indicate that the gut microbiome plays a very important role in this disease in which Lactobacillus in the gut has been shown to regulate glucose and lipid metabolism, improving oxidative stress, and inhibiting inflammatory responses.

Previous studies showed that remifentanil-induced anesthesia can inhibit surgical stress response in non-diabetic adult patients and that low-dose glucose loading during anesthesia may attenuate fat catabolism. However, little is known about the influence of glucose loading on metabolism in elderly patients, whose condition may be influenced by decreased basal metabolism and increased insulin resistance.

The surgical resection of lung disrupts glucose homeostasis and causes hyperglycemia, as in any other major surgery or critical illness. We performed a prospective study where we carefully lowered hyperglycemia by insulin administration during the surgery, and for the first time we monitored immediate insulin effects on lung physiology and gene transcription.
