Transoral surgery

Over the past decade, therapeutic options in head and neck supraglottic squamous cell carcinoma have constantly evolved. The classical total laryngectomy has been partially replaced by alternative organ- and function-sparing techniques with the same prognosis but less morbidity, such as Radiotherapy, Transoral Laser Microsurgery (TLM) and Trans-Oral Robotic Surgery (TORS).

The revision surgery of basilar invagination (BI) with irreducible atlantoaxial dislocation (IAAD) after a previous occipitocervical fusion (OCF) is challenging. Transoral revision surgery has more advantages than a combined anterior and posterior approach in addressing this pathology.

Human papillomavirus-positive oropharyngeal squamous cell carcinoma is increasing in incidence worldwide. Current treatments are associated with high survival rates but often result in significant long-term toxicities. In particular, long-term dysphagia has a negative impact on patient quality of life and health.

For loco-regionally advanced, but transorally resectable oropharyngeal cancer (OPSCC), the current standard of care includes surgical resection and risk-adapted adjuvant (chemo) radiotherapy, or definite chemoradiation with or without salvage surgery.

Clinical outcomes of transoral videolaryngoscopic surgery for hypopharyngeal and supraglottic cancer
Transoral videolaryngoscopic surgery (TOVS) was developed as a new distinct surgical procedure for hypopharyngeal cancer (HPC) and supraglottic cancer (SGC) staged at up to T3. However, long-term treatment outcomes of TOVS remain to be validated.

Patients with human papillomavirus-positive (HPV+) oropharyngeal squamous cell carcinoma (OPC) have substantially better treatment response and overall survival (OS) than patients with HPV-negative disease. Treatment options for HPV+ OPC can involve either a primary radiotherapy (RT) approach (± concomitant chemotherapy) or a primary surgical approach (± adjuvant radiation) with transoral surgery (TOS).

is with great pleasure and honor that I embrace the opportunity to write this book. After more than 25 years of practice, clinical observation, and case documentation, you get excited about what works and what does not work. The case study format is an excellent way to share this knowledge. All the cases are presented in a systematic format illustrating examination, diagnosis, diagnostic wax - up, treatment plan, treatment sequence, provisionalization, and the fi nished case.

A laser, an acronym lor light amplification by stimulated emission of radiation, is a device for generating a high-intensity, ostensibly parallel beam of monochromatic (single wavelength) electromagnetic radiation. The possibility of stimulated emission was predicted by Einstein in I9I7; based on the work of Gordon in 1955 and Schawlow and Townes in 1958, Maiman created the first operational laser in 1960, a ruby laser emitting a brilliant red beam of light.
