To evaluate long-term results of treatment for avascular necrosis of femoral head in late stages by using Spiron total hip replacement. Subject and method: From 2012 to 2016, 68 patients who are not older than 50 with stage IV, V, VI of avascular necrosis (According to Steinberg classification) were treated by 90 Spiron hips replacement in the Institute of Trauma and Orthopedic - 108 Military Central Hospital.

Bone infections due to trauma and subsequent delayed or impaired fracture healing represent a great challenge in orthopedics and trauma surgery. The prevalence of such bacterial infection-related types of delayed non-union is high in complex fractures, particularly in open fractures with additional extensive soft-tissue damage.

During the last three decades, important advances have been made in the available treatments for the loss of skeletal tissue as a result of trauma or disease. The application of large skeletal allografts and total joint replacement have become successful and reproducible treatment options.
