Twin pregnancy

Twin pregnancies are an economically unwanted phenomenon in dairy cattle, not only because they increase pregnancy losses, but also because antibiotics usage and culling rate of the dam are also dramatically increased due to them, furthermore animal welfare issues are also affected through them.

We present the first case to our knowledge of a spontaneous twin pregnancy in a 16-year-old Caucasian patient with cystic fibrosis and systemic lupus erythematosus. Cystic fibrosis is one of the most common autosomal recessive genetic disorders and primarily affects the respiratory and digestive systems.

Compared with singleton pregnancy, twin gestation is featured by a greater increase in cardiac output. Therefore, norepinephrine might be more suitable than phenylephrine for maintaining blood pressure during cesarean section for twins, as phenylephrine causes reflex bradycardia and a resultant decrease in cardiac output.

Obstructive sleep apnoea (OSA) is related with adverse pregnancy outcomes, including preeclampsia. However, there are small studies about treatment of OSA with automatic continuous positive airway pressure (CPAP) in adverse obstetric outcomes.

Current knowledge indicate that epirubicin administration in late pregnancy is almost devoid of any fetal cardiotoxicity. We report a twin pregnancy complicated by breast cancer in which epirubicin administration was causatively linked to the death of one twin who was small for gestational age (SGA) and in a condition of oligohydramnios and determined the onset of a transient cardiotoxicity of the surviving fetus/newborn.

Dystocia reported in four goats were diagnosed with mummification of either single or twin foetuses. Mummification of foetuses was confirmed by per-vaginal examinations. One case was diagnosed as papyraceous type and other three cases were haematic type of mummification.

A murrah buffalo in third parity was presented in eighth month of gestation with the history of vaginal discharge from past two days. Per vaginal investigations done to diagnose the etiology suggested fully dilated cervix with the presence of twin fetuses. Taking into consideration, the unsuccessful delivery attempts by the animal, it was decided to obstetrically intervene, which led to the birth of dead male co-twins.

Twin-to-twin transfusion syndrome (TTTS) is a serious complication of 10–15% of twin or triplet pregnancies in which multiple fetuses share a single placenta. Communicating placental vessels allow one fetus (the donor) to pump blood to the other (the recipient).

Objective: In 2007 and 2008, the Japan Society for Reproductive Medicine and the Japan Society of Obstetrics and Gynecology issued a recommendation for single embryo transfer (SET). Thereafter, SET was implemented in 73% of in vitro fertilization (IVF) cases in Japan. The purpose of this study was to evaluate the effects of compliance with the SET recommendation on perinatal outcomes.

Ultrasound techniques are becoming increasingly important in animal reproduction, offering both a mean of diagnosis and a useful therapeutic tool. Accordingly, understanding the use of ultrasound technology is critical in contemporary animal sciences, since ultrasound examinations are now a routine component of diagnostic workups in reproduction.

As recently as the early 1980s, twin pregnancy and birth was a relatively rare event, and higher-order multiples were of negligible consequence. However, this did not deter clinicians as well as researchers from evaluating the clinical management of multiples. Obviously, the validity of many of these studies was hampered because of small sample size and underpowered statistics. Beginning in the early 1980s, all we knew about the natural history of multiples has been profoundly changed.

Breech is a longitudinal presentation in which the cephalic pole occupies the fundus and the caudal (podalic) pole lies in the lower segment of the uterine cavity or within the birth canal. Overall breech presentation occurs in 3%–4% of singleton pregnancies commencing labor, but has a much higher incidence in multiple gestations (e.g., 25% of first twins and 50% of second twins are breech). The incidence rises further in higher order multiple pregnancies.

Multiple pregnancy involves more than one embryo (fetus) in any one gestation. Two independent mechanisms may lead to multiple gestation: segmentation of a single fertile ovum (identical, monovular, or monozygotic) or fertilization of separate ova by different spermatozoa (fraternal or dizygotic) multiple pregnancy. In the development of twins (the most frequent higher-order gestation), monozygotism is constant ( 2.3–4/1000 deliveries), whereas dizygotism has certain predispositions. Dizygotic twinning is inherited as a recessive autosomal trait via the female descendants.
