Survival of cervical cancer patients at Moi teaching and Referral Hospital, Eldoret in western Kenya
Cervical cancer is a major health burden and the second most common cancer after breast cancer among women in Kenya. Worldwide cervical cancer constitutes 3.1% of all cancer cases. Mortality rates are greatest among the low-income countries because of lack of awareness, screening and early-detection programs and adequate treatment facilities.

Adolescents living with HIV (ALHIV, ages 10–19) experience complex barriers to care engagement. Challenges surrounding HIV status disclosure or non-disclosure to adolescents may contribute to adolescent disengagement from HIV care or non-adherence to ART. We performed a qualitative study to investigate the contribution of disclosure challenges to adolescent disengagement from HIV care.

Despite guidelines for cervical cancer prevention in low-resource countries, a very small proportion of women in these settings undergo screening, and even fewer women are successfully treated. Using pilot data from western Kenya and World Health Organization recommendations, we developed a protocol to implement evidence-based cervical cancer screening and linkage to treatment strategies to the rural communities.

USAID’s APHIA plus platforms; CDC’s on-going population-based surveillance such as the western Kenya Health and Demographic Surveillance System (HDSS) and the Kibera-based population-based surveillance site; and the Dadaab and Kakuma refugee camps (of note, refugees are often excluded from national policies and programs, to the detriment of the health of refugees and their host communities). These platforms, in particular the population-based surveillance platforms, offer the capacity to precisely measure inputs and outcomes, including interim and proxy indicators.

Women with a positive VIA test in the pilot or early expansion phase, or a positive VILI test in the late expansion phase, were considered screen-positive and were counseled to go to the district hospital for further evaluation (“triage”) to determine what management was needed. Test results including a diagram of the lesion location on the cervix were noted on specially designed, client-retained cards (see Appendix 3) to be shown at the district hospital.

The constraints of limited infrastructure and resources in most developing countries and the low level of awareness of opportunities for preventing the disease stimulated the formation in 1999 of the international Alliance for Cervical Cancer Prevention (ACCP), with funding from the Bill & Melinda Gates Foundation. The purposes of the ACCP are to develop and evaluate innovative approaches in order to reach more women at high risk of cervical cancer with effective and feasible screening and treatment services and to persuade policymakers and program managers to make it a priority.

The Western Kenya Cervical Cancer Prevention Project (WKCCPP), implemented from 2000 to 2004, was a collaborative project to develop and evaluate a model cervical cancer prevention program suitable for rural, low-resource settings in Africa. The Ministry of Health (MOH), Maendeleo ya Wanawake Organization (MYWO), and the Kenya Cancer Association (KECANSA) were partners with PATH in the project.
