To characterize emerging and current practice models to more effectively treat and support patients with multiple chronic conditions (MCC). Integrating care for MCC patient populations requires processes for determining different subpopulation needs in various settings and lived experiences. Innovation should be anchored at the nexus of payment systems, social risks, medical needs, and community-based resources.
