The evidence presented in this report shows that the health of our educators is a source of concern because the prevalence of HIV is high. High rates of chronic conditions, dementia, frailty or disability, and sub-optimal home environment and social supports call for the integration of primary care and case management for elderly patients of community health centers. Case management may support health center disease management efforts but is primarily aimed at supporting the coordination of services that are necessary for living safely in a home environment.
