New view: “An accident is any unplanned event that results in injury or ill health of people or damage or loss to property, plant materials or the environment or loss of a business opportunity” Accident 1 • A 37 years old man had both 2 legs clamp in the soil grinder. • With the help form doctors, two- thirds of the victim’s body was taken out of the machine, but 1 leg was still stuck in the machine. 2

  4. 3/3/2016 Terminology Definition • A “near miss” is an unplanned event that did not result in injury, illness or damage - but had the potential to do so. • Sometimes called a “near hit” or “close call” – signals a system weakness that if not corrected could lead to significant consequences in the future. 7 Terminology Watch examples and do discuss 1.   9. 3/3/2016 Terminology RISK ??? Risk: chance or probability that a person will be harmed or experience an adverse health effect if exposed to a hazard. Example: the risk of developing cancer from smoking cigarette Terminology RISK ??? Influence factors of risk include: • how much a person is exposed to a hazardous thing or condition, • how the person is exposed (e.g., breathing in a vapour, skin contact), and • how severe are the effects under the conditions of exposure 9
