Acute lower respiratory infections

Identifying trends of hospital admissions for respiratory diseases is crucial for public health and research to guide future clinical improvements for better outcomes. This study aims to define the trends of respiratory disease-related hospital admissions (RRHA) in England and Wales between 1999 and 2019.

Many patients with bronchiectasis have recurrent hospitalisations for infective exacerbations. Acute kidney injury (AKI) is known to be associated with increased in-hospital mortality. This study examined the frequency of AKI, associated risk-factors, and the association of AKI with in-hospital mortality among patients with bronchiectasis.

Lower respiratory tract infections are one of the leading causes of morbidity and mortality in children worldwide. Several viruses can cause bronchiolitis. Respiratory syncytial virus (RSV), parainfluenza viruses (PIV), influenza type A, adenovirus and human metapneumovirus (HMPV) are some of the common viruses found in hospital studies. This study aimed to study the molecular diagnosis of HPMV infections by using a realtime, reverse-transcriptase PCR assay and the evaluation of its prevalence among Egyptian infants clinically diagnosed with acute viral bronchiolitis.

Human bocavirus (HBoV) was first identified as parvovirus in 2005 in nasopharyngeal aspirates of children with lower respiratory tract infection. The objectives of this study were to determine HBoV as a cause of acute lower respiratory tract infection(ALRTI) in children by investigating the presence of HBoV DNA, to evaluate diagnostic efficiency of HBov IgM assay in the serum of the studied patients and to assess the impact of the viral load on the clinical manifestations of children with positive HBoV DNA samples.

Acute lower respiratory infections (ALRIs) are leading causes of hospitalisation in children. Birth defects occur in 5% of live births in Western Australia (WA). The association between birth defects and ALRI hospitalisation is unknown.

Pneumonia and acute bronchitis are the most common Lower Respiratory Tract Infections (LRTIs). They are caused by a variety of organisms. 472 BAL samples collected from clinically suspected lower respiratory tract infections were processed in the microbiology laboratory. Of the total samples, males constituted 336 (71.2%) and 136 (28.8%) females. 223 (47.2%) of the samples were of age group 40-60 years. 383 (81.1%) of samples showed growth of various bacteria. Fungal isolation was seen in 21 (4.4%) of samples. This study shows various pathogens isolated from lower respiratory tract infections.

The comparison of the frequencies of bacterial and viral infections among children with communityacquired pneumonia (CAP) admitted in distinct severity categories, in an original study, is lacking in literature to-date. We aimed to achieve this goal.

Acute lower respiratory tract infection is the commonest disease affecting children under five worldwide. Respiratory syncytial virus (RSV) is among the most common causative pathogens. Epidemiological data suggest an association between severe viral respiratory infections in infancy and increased incidence of childhood wheeze and asthma.

Respiratory Syncytial Virus (RSV) causes pediatric pneumonia and lower respiratory tract infection (LRTI) in older adults and immunocompromised individuals. The Present study aims to determine the proportion of RSV infection in patients with acute LRTI. Two step quantitative reverse transcriptase real time PCR was carried out on 80 samples for detection of RSV. The proportion RSV infection in our patients was found to be 1.25%. Our study showed that RSV infection is not a common cause of LRTI in adults in this area.

Respiratory syncytial virus (RSV) is the most frequently identified pathogen in children with acute lower respiratory tract infection. Fatal cases have mainly been reported during the first 6 months of life or in the presence of comorbidity.

There’s abundant sunshine in the tropics but severe rickets is still observed. Nutritional rickets is associated with an increased risk of acute lower respiratory infections. Pneumonia is the leading cause of death in the under 5 -year old children with the highest burden in developing countries.

Chronic obstructive pulmonary disease (COPD) is a spectrum of airway diseases that has chronic bronchitis at one end and emphysema at other end. COPD is the major cause of morbidity and mortality worldwide. Most of the episodes (80%) of AECOPD are triggered by infections mainly affecting the lower respiratory tract by respiratory viruses, atypical bacteria and aerobic Gram positive and Gram negative bacteria.

NO is a gaseous molecule, synthesized by NO synthase (NOS), an enzyme that catalyses the oxidation of L-arginine to NO and L-citrulline. At least two types of NOS can be reported, constitutive NOS (cNOS) and inducible NOS (iNOS) (168) . cNOS is produced by many cells in the upper and lower respiratory system, such as parasympathetic vasodilator nerves, endothelial cells and ciliated mucosa cells (169) .

Ultimately, most household energy interventions - directly or indirectly - aim to improve health among their target populations. This module consists of one presentation and one practical exercise that are concerned with methods available to monitor the impact of interventions on children's and women's respiratory health and overall well-being. A review of the evidence for the linkages between indoor air pollution, household energy and health provides the introduction to this module (Figure 5).

An estimated 1.5 million deaths occur annually as a result of household air pollution from SFU mainly for cooking as well as winter season heating. The total disease burden, including morbidity, is estimated at 36 million DALYs (WHO 2007). 3 These deaths and DALYs arise mainly from acute lower respiratory infections (ALRI) in young children and chronic obstructive pulmonary disease (COPD) in adults, and to a lesser extent lung cancer. There is also moderate evidence of increased risk of asthma, cataracts and tuberculosis (Desai et al, 2004; Smith et al, 2004).

Population displacement caused by cyclone damage and flooding can result in overcrowding in resettlement areas, raising the risk of transmission of certain communicable diseases. Measles (see section below on vaccine-preventable diseases), ARI, diphtheria and pertussis are transmitted from person to person through respiratory droplets, and the risks are increased in situations of forced relocation to shared areas which are overcrowded and have inadequate ventilation. Overcrowding can also increase the likelihood of transmission of meningitis, waterborne and vector-borne diseases.

Multiple serotype infection was found to be more frequent in immunocompromised patients (30%) than in immunocompetent patients (5%) (Gray et al., 2007). Our group also experienced sequential adenovirus infection of serotype 14 hemorrhagic cystitis and serotype 35 disseminated infection after cord blood transplantation (Abe T et al., 2009). Serotype 14 is a group B adenovirus, which is associated with pharyngo-conjunctival fever (Tuck et al., 1957).

Influenza is an acute respiratory illness caused by infection with influenza viruses. The illness affects the upper and/or lower respiratory tract and is often accompanied by systemic signs and symptoms such as fever, headache, myalgia, and weakness. Outbreaks of illness of variable extent and severity occur nearly every winter. Such outbreaks result in significant morbidity in the general population and in increased mortality rates among certain high-risk patients, mainly as a result of pulmonary complications.
