Strains producing extendedspectrum beta-lactamases (ESBLs) or carbapenemases are considered global priority pathogens for which new treatment and prevention strategies are urgently required, due to severely limited therapeutic options.

Antimicrobial resistance (AMR) is an increasing global health concern reducing options for therapy of infections and also for perioperative prophylaxis. Many Enterobacteriaceae cannot be treated anymore with third generation cephalosporins (3GC) due to the production of certain 3GC hydrolysing enzymes (extended spectrum beta-lactamases, ESBLs).

About 150 million people suffer from urinary tract infection each year. In majority of cases, treatment is initiated empirically based on the antimicrobial resistance pattern of the urinary pathogens prevalent in a particular setting. Emerging antibiotic resistance among Enterobacteriaceae has posed challenges in choosing empiric regimens. Therefore, the present study was designed to identify etiological agents of urinary tract infections, detect ESBL producing uropathogens and study their antibiotic resistance profile.

This study was focused to investigate the prevalence of different uropathogens producing ESBL and its antibiogram in non-hospitalized patients. 1495 non-repetitive urine samples were collected in the study. A total of 335 (22.41%) samples showed significant bacteriuria.

The drug resistance among the gram negative bacteria is present as a serious global problem. ESBLs and MBLs are the important members of beta-lactamases produced mainly by gram negative bacteria and are responsible for mediating resistance to extendedspectrum cephalosporins and carbapenem. These enzymes are commonly detected in the members of the Enterobacteriaceae like Klebsiella pneumoniae and Escherichia coli. A cross-sectional study was conducted at Department of Microbiology, Santosh Medical College & Hospital Ghaziabad by using K. pneumoniae isolated from sputum samples.
