Gastric emptying

Patients undergoing gastroenteroscopy during sedation are prone to aspiration, and most patients with dyspepsia have delayed gastric emptying. This study aimed to investigate the feasibility of measuring the gastric antrum cross-sectional area (CSA) to supply a novel clinical diagnostic reference value in patients with dyspepsia.

Surgical patients are asked to fast for a sufficient duration to ensure that the amount of residual liquid in the stomach is within the safe range, thereby reducing the risk of gastric reflux perioperatively. The authors hypothesized that subjective hunger numerical rating scale (NRS) score could also help assess the process of gastric emptying and determine the amount of fluid remaining in the stomach.

Delayed gastric emptying and the resultant “full stomach” is the most important risk factor for perioperative pulmonary aspiration. Using point-of-care gastric sonography, we aimed to investigate the prevalence of full stomach and its risk factors in elective surgical patients with type 2 diabetes.

Polytrauma patients are at a higher risk of delayed gastric emptying. To assess the gastric volume, a reliable diagnostic tool is needed to prevent the occurrence of aspiration pneumonia, which remains a serious complication associated with anesthesia.

Postoperative nausea and vomiting and postoperative ileus are common after major digestive surgery and represent one of the significant problems in Acute Care Surgery. The delivery model of emergency surgery needs to be improved in order to foster a patient-centered care. The multimodal approach suggested by Enhanced Recovery After Surgery (ERAS®) Guidelines is gaining widespread acceptance but is difficult to apply to emergency surgery.

(BQ) For this reason this version differs from previous editions of Yamada’s Handbook by providing Key Practice Points, easily identified in “call-out boxes” in each chapter, which highlight the most important factors that guide clinical care. The case scenarios created for each chapter in Part 1: “Approach to Patients with Gastrointestinal Symptoms” are accompanied by discussions that we hope will provide the context necessary to translate medical knowledge to clinical practice.

Laparoscopic-assisted pylorus-preserving gastrectomy (LAPPG) is a minimally invasive functionpreserving surgery for early gastric cancer. This study was designed to investigate the clinical outcomes between LAPPG and laparoscopy-assisted distal gastrectomy (LADG) by objective evaluation.

The incretin hormones GLP-1 and GIP are released from the gut during meals, and serve as enhancers of glucose stimulated insu-lin release from the beta cells. Furthermore, GLP-1 also stimu-lates beta cell growth and insulin biosynthesis, inhibits glucagon secretion, reduces free fatty acids and delays gastric emptying. GLP-1 has therefore been suggested as a potentially new treat-ment for type 2 diabetes.

A pregnant woman is considered to always have a “full stomach” due to delayed gastric emptying and incompetent lower esophageal sphincter. Thus, pregnant women are at increased risk for aspiration (13;14). Therefore, prophylactic measures to prevent aspiration should be used, particularly during the third trimester. A woman with multiple gestation is at increased risk for aspiration in the mid-second trimester because of the large uterus. Maintaining a semi-seated position and avoiding excessive sedation are required to prevent aspiration.

The GIT motility constituents such as migrating motor complex in the stomach and the one way peristaltic movements in small intestine are significantly influenced by the bacterial colonization. Moreover, the decrease of intestinal motility may cause a small intestinal bacterial overgrowth (SIBO).

It is generally accepted that enteral feeding is preferable to parenteral feeding for critically ill patients, since it reduces mortality, it decreases the number of complications and of course it is much cheaper than parenteral nutrition (Box 1.1). However, achieving targets for feeding remains problematic since delayed gastric emptying, common in such patients, can be a cause of feeding cessation. This article discusses the physiological mechanisms of delayed gastric emptying, the ways in which it can be assessed, and what can be done to remedy matters....

Physical Features Patients with AN typically have few physical complaints but may note cold intolerance. Gastrointestinal motility is diminished, leading to reduced gastric emptying and constipation. Some women who develop AN after menarche report that their menses ceased before significant weight loss occurred. Weight and height should be measured to allow calculation of body mass index (BMI; kg/m2). Vital signs may reveal bradycardia, hypotension, and mild hypothermia. Soft, downy hair growth (lanugo) sometimes occurs, and alopecia may be seen.
