Interscalene block (ISB) is commonly associated with Horner’s syndrome due to spread of local anesthetic to the cervical sympathetic chain. Postganglionic neurons that originate from superior cervical ganglia form the sympathetic innervation of eye. Decrease in sympathetic tone may change intraocular pressure (IOP) and ocular perfusion pressure (OPP). The aim of the study was to investigate whether ISB affects IOP and/or OPP.

Attenuation of an increase in intraocular pressure (IOP) is crucial to preventing devastating postoperative visual loss following surgery. IOP is affected by several factors, including the physiologic alteration due to pneumoperitoneum and patient positioning and differences in anesthetic regimens. This study aimed to investigate the effects of propofol-based total intravenous anesthesia (TIVA) and volatile anesthesia on IOP. A case is reported of acute bilateral myopia and angle closure glaucoma in a 7-year-old patient from topiramate toxicity. This is the second known reported case of topiramate induced acute angle closure glaucoma and third known reported case of topiramate induced acute myopia in a pediatric patient.

Glaucoma results in "cupping" as the neural rim is destroyed and the central cup becomes enlarged and excavated. The cup-to-disc ratio is about 0.7/1.0 in this patient. In acute angle-closure glaucoma, the eye is red and painful due to abrupt, severe elevation of intraocular pressure. Such cases account for only a minority of glaucoma cases: most patients have open, anterior chamber angles. The cause of raised intraocular pressure in open angle glaucoma is unknown, but it is associated with gene mutations in the heritable forms.

Introductio: β-Blockers (β-adrenergic receptor antagonists) block the effects of catecholamines on signal transmission through β-receptors; they cause hypotension by decreasing the heart beat rate and cardiac output, and prevent the attacks of arrhythmia and angina pectoris. In the brain, the drugs attenuate migraine by suppressing the dilation of blood vessels; in the eye, they decrease intraocular pressure by suppressing the production of aqueous humor.
