Therapeutic blockade of tumour necrosis factor alpha (anti-TNF-α) is the mainstay treatment of several rheumatologic diseases. They are unfrequently associated with opportunistic infections and latent tuberculosis (TB) screening is paramount before immunosuppression. A 62-year-old man with psoriatic arthritis was under treatment with adalimumab for over 5 years.

Fish oils derived from oily fish are an excellent source of omega -3 fatty acids. Omega 3 fatty acids are poly unsaturated fatty acids (PUFA) and are essential fatty acids (EFA), as humans cannot synthesise them de novo and must depend on dietary sources. Data suggests a role of omega-3 EFA in the prevention and treatment of gastrointestinal, rheumatologic, bone and respiratory illness.

Self-management of inflammatory arthritis (IA) requires patients to address the impact of symptoms, treatment, and the psychosocial consequences of a long term condition. There are several possible mechanisms for facilitating self-management, including patient-clinician interactions in routine consultations.

In recent years, the treatment of Rheumatoid Arthritis has undergone tremendous changes due to methotrexate and biologics in large part. Until a decade ago, avoiding joint destruction in rheumatoid arthritis was not realistic for rheumatologist. However, prevention of joint deterioration is now possible, and the quality of life of patients with rheumatoid arthritis in long-term has been able to improve by advances in treatment.

Rheumatoid arthritis is a chronic systemic inflammatory arthritis of auto-immune origin that affects primarily the synovial joints, usually in a symmetrical pattern. It is the most common and most serious of the inflammatory arthritides, and it dominates clinical rheumatological practice (Silman 2002). Current evidence shows that prompt diagnosis and early instigation of definitive disease-modifying treatment can delay or avoid progression and enable patients to retain function that would otherwise be lost in this progressive and frequently disabling disease....

We started the treatment by giving alpha-blocker. His PSA at first visit was 4.20 ng/mL, and became slightly elevated to 5.47 ng/mL after two months. Transrectal prostate biopsy revealed prostate cancer confined in the right lobe. Gleason scores were 6 (3 + 3) in two out of 14 cores. He underwent standard TURP of the transition and central zone, and then we made a deeper resection of the peripheral zone of the right lobe. The operation took 80 minutes with no blood transfusion and water intoxication, and the resected weight was 27.0 g.
