Vitamin D insufficiency

To establish the prevalence of vitamin D deficiency and insufficiency in healthy South Korean children aged between 1 month and 16 years and to identify factors affecting the serum vitamin D levels in South Korea. A total 969 healthy children were enrolled in this study.

Changes in calcium metabolism and calcium urinary excretion during chemotherapy have not been thoroughly assessed in patients with early breast cancer (EBC), a population who frequently present vitamin D insufficiency. Osteoporosis and fractures may increase due to hypoestrogenism in menopause and cytochrome P450 inducing AEDs. Recent studies suggest lower bone mineral density (BMD) in adults and children with epilepsy, irrespective of AED treatment. Both idiopathic epilepsy and symptomatic epilepsy are associated with reduced BMD, with the greatest reduction in symptomatic generalized epilepsy (Sheth & Hermann, 2008). However, the pathophysiological underlying mechanisms are far from understood and likely multifactorial.

Vitamin D deficiency, impaired 1,25(OH)2D production (primarily secondary to renal insufficiency), or, rarely, vitamin D resistance also cause hypocalcemia. However, the degree of hypocalcemia in these disorders is generally not as severe as that seen with hypoparathyroidism because the parathyroids are capable of mounting a compensatory increase in PTH secretion. Hypocalcemia may also occur in conditions associated with severe tissue injury such as burns, rhabdomyolysis, tumor lysis, or pancreatitis.
